The physician finder database is available via the Pelvic Awareness Project and is offered as a resource to help patients locate specialists in their area. The information included in the database is based on your voluntary response, and requires no purchase or fee to participate. The listing is not exhaustive. This database does not include all physicians who practice in the field of Gynecology, Urology or Urogynecology. Inclusion of your information in the database does not represent an endorsement, referral, or recommendation of you or your practice from Pelvic Awareness Project. Inclusion of your information also is not dependent on any historical or future purchase from the Pelvic Awareness Project or Caldera Medical. In addition, your participation in the database does not constitute an endorsement of the Pelvic Awareness Project. The Pelvic Awareness Project Physician Finder uses a third party, HIPAA compliant, call tracking numbers with metrics allowing us to report to your practice the number of calls you are receiving per month. Your self-selection and the accuracy of the responses to be included in the database is your responsibility and you can request to have your name and information removed at any time by contacting [email protected].
